Medical Case Management

Medical case management begins the moment a positive client comes to APGD, whether through our testing program or  from somewhere else.

Our medical case managers Josie and Evelyn offer services in English and Spanish and spend their days ensuring their clients are well cared for. They work with all other parts of the agency and outside providers to book clinic and medical appointments, follow up on blood work and meet in case conference to ensure a comprehensive treatment plan is adhered to.

Medical case managers meet regularly with their clients to determine needs related to the various challenges that stand in the way of positive health outcomes. This can include referral to housing for help with keeping a roof over their head, taking them to the food pantry, linking them to mental or substance abuse counseling or navigating the complex healthcare landscape
